Fab-fractie

Modaliteit

Antistoffragmenten, voorgoed door het antigen-bindend fragment (Fab), vormen een alternatief voor conventionele immunoglobulines (IgGs) in de gebieden van diagnostiek en therapie bij verschillende ziekten. Hun opvallende voordelen omvatten verbeterde weefseldoordringing door een lagere moleculaire gewicht ten opzichte van IgG, verminderd ontstekingspotentieel toegeschreven aan het ontbreken van het Fc-domein, en efficiënte productie gefaciliteerd door lage structurele complexiteit. Klinisch hebben verschillende Fab-fragmenten, waaronder Abciximab, Ranibizumab, Certolizumab Pegol en Idarucizumab, therapeutische goedkeuring verkregen, wat hun effectiviteit en relevantie in medische toepassingen onderstreept.

Fab Fragment voor therapeutisch gebruik

Abciximab

Abciximab (Reopro) is een chimerische monoclonale antilichamen Fab-fractie, combinerend menselijke en muizen-elementen. De werking van abciximab houdt in dat de aggregatie van bloedplaatjes wordt belemmerd door antagonisering van de GP IIb/IIIa receptor.

Ranibizumab

Het tweede door de FDA goedgekeurde anti-VEGF medicijn, ranibizumab (Lucentis, ontwikkeld door Genentech/Novartis), is een antilichaamsfragment met affiniteit voor alle VEGF-isovormen. Pivoteel Fase III onderzoek naar ranibizumab, ANCHOR en MARINA, toonde niet alleen visuele stabilisatie maar ook aanzienlijke visuele verbeteringen, wat een belangrijke mijlpaal vormde in klinische uitkomsten.

Certolizumab Pegol

Certolizumab pegol, toegediend via subcutane injectie, functioneert als een remmer van tumor necrose factor-alpha (TNFα) activiteit. Het remt effectief aanhoudende ontsteking bij personen die zijn gediagnosticeerd met Crohn ziekte.

Idarucizumab

Idarucizumab, een humaniseerd monoclaal antibodyfragment, is zorgvuldig ontworpen om te richten op dabigatran, met een bindingsefficiëntie aan de thrombinsite die 350 maal groter is dan die van thrombin zelf. Deze uitzonderlijke binding leidt tot bijna onomkeerbaar binden totdat nierexcretie plaatsvindt. Idarucizumab heeft goedkeuring verkregen voor zijn effectiviteit in het omkeren van de anticoagulerende werking van dabigatran.
